Declaration of Consent

I, below consenting to this form, hereby give to the personal data controller, company Arthur Hunt, s.r.o., with its registered office at Anny Letenské 34/7, 120 00 Praha 2, Vinohrady, IČ: 618 75 882 (hereinafter “AH”), in conformity with the Act No. 101/2000 Coll. and Act No. 480/2004 Coll., as amended, and in conformity with relevant European union regulation, in particular General Data Protection Regulation, i.e. Regulation (EU) 2016/679 of the European parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC, so called Regulation GDPR, my consent to the processing of the personal data in the following scope:

  • name, surname,
  • date of birth,
  • residence,
  • phone number,
  • e-mail address,
  • marital status, information about achieved education, working experience and completed trainings,
  • as well as sensitive personal data, precisely the data revealing my criminal activity,

(hereinafter the “personal data”), for the purpose of:

  • active searching candidates for filling jobs offered by clients AH;
  • for the purpose of recruitment procedure;

I agree that my personal data will be processed for the period which shall expire after 10 years from the day I give this consent.

At the same time, I declare that personal data I have provided are accurate and truthful and I expressly agree that personal data in its entirety and for the purposes above mentioned would be processed by third parties authorized by the administrator of personal data, the company AH, especially legal entities belonging to the ARTHUR HUNT Human Resources Consulting Group and by corporations who are with AH in a contractual relationship, in line with the activity of the company AH, which is recruitment.

Revocation of consent is effective at the moment of delivery to AH. Revocation of consent is without prejudice to the lawfulness of processing based on consent until the time of its withdrawal.

AH may delegate the processing of personal data to third parties as processors and personal data may be transferred to other countries.

The legal basis for processing is the necessity for fulfilling the contractual relationship with the data subject.

AH company will process personal data both manually and automatically by its own employees. I declare that I voluntarily provide my personal data and I was familiar with my right in particular:

  1. to require access to my personal data from AH;

I have the right to obtain from AH a confirmation that personal data concerning me is processed or not and right to gain access to these data. AH is required to provide a copy of the processed personal data. AH is entitled to claim a reasonable fee based on administrative costs for further copies of on my request.

  1. correcting inaccurate data and also completing incomplete personal data taking into account the purpose of processing;
  2. the deletion of personal data (the “right to be forgotten”) or the restriction of processing under specified conditions, in particular, where personal data are no longer needed for the purposes for which they were collected or otherwise processed and there is no further legal reason for processing;
  3. a data transferability
  4. I have the right to obtain personal data concerning myself that I have provided to AH in a structured, commonly used and machine-readable format, the right to pass this information to another controller. In exercising this right, I may request that personal data be passed directly from AH to another controller if technically feasible.
  5. to object to processing for marketing purposes;

If I object to processing for marketing purposes, AH will terminate the processing of personal data for this purpose.

  1. file a complaint with the Supervisory Authority, i.e. the Office for Personal Data Protection (especially if I have doubts about compliance with rights of AH as a personal data controller);
